The structural engineer places a column exactly where the interior designer placed the kitchen island. The electrical layout assumes a ceiling height the architect quietly lowered two revisions ago. Nobody lied. Everyone kept working, correctly, from their own — slightly outdated — version of the truth.

Each drawing is internally consistent. Each discipline can defend every line on it. The problem only exists in the gap between the drawings — and nobody owns that gap until someone is made to.
